SECTION- 5
Sub sec 5.1(a) following documents are required for application-
Ø Valid Identity Proof (Aadhaar Card / Passport / Ration Card/ PAN Card)
Ø Valid Proof of Date of Birth (Passport/ School Leaving Certificate / Transfer Certificate/ Aadhaar Card on which DOB is mentioned)
Ø Valid Proof of Residence
Ø Academic qualification mark sheet and certificate including medical qualification
Ø Passport size colour photo of the candidate
Ø Internship completion certificate
Sub sec 5.1(b) following fees to be required for application-
Fees- R.S. 5500/- (t & c applicable, it can changeable according to notice of the council time to time)
Sub sec 5.2 (a) after completion the internship; candidate (council’s students only) can apply to council for their practice registration with proper documents and fees as per sub-section 5.1 (a) and sub-section 5.1 (b)
Sub sec 5.2 (b) after completion the course, candidate (council’s students only) can apply to council for their practice registration with proper documents and fees as per sub-section 5.1 (a) and sub-section 5.1 (b)
Sub sec 5.2 (c) If any other candidate wishes to apply for registration of practice, they may apply for registration of practice to this Council with appropriate documents as per sub-section 5.1 (a) and sub-section 5.1 (b) along with N.O.C. from their institute (T & C applicable).
Sub sec 5.2 (d) if any other candidate wishes to apply for registration of practice, they may apply for registration of practice to this Council with appropriate documents as per sub-section 5.1 (a) and sub-section 5.1 (b) along with minimum 5 years’ experience certificate from any regd. doctor.
Sub sec 5.2 (e) If any other candidate (who practicing as a health care provider for long time) wishes to apply for registration of practice, they may apply for registration of practice to this Council with appropriate documents as per sub-section 5.1 (a) and sub-section 5.1 (b) along with character certificate or N.O.C. from their panchayet / municipality / M.L.A. / M.P. / gazetted officer or any other Govt. officer.
Sub sec 5.3 (a) grade ‘A’ registration will be provided to only for Sub sec 5.2 (a), Sub sec 5.2 (b), Sub sec 5.2 (c)
Sub sec 5.3 (b) grade ‘B’ registration will be provided to only for Sub sec 5.2 (d), Sub sec 5.2 (e).
Sub sec 5.4 after application, it will take 30-60 days to confer certificate to candidate.
Sub sec 5.5 after getting the registration certificate they should practice with said medicinal system only. Otherwise any Govt. or this council can cancel their certificate without notification if they cannot prove to be honest their practice.
